Before you start

This form takes about three minutes. It tells us how and when you would like to meet a notary.

The notary appointment is where your identity is checked and where you sign the limited power of attorney. We confirm a notary within 24 hours after you submit, and the notary will reach out to you directly.

Do not type ID numbers and do not upload documents here. Questions? Call us at (888) 545-8007.

Step 1 of 8: About you
    1About you
Type your name exactly as it is printed on your government photo ID. The notary will compare the two.
We and the notary use this number to set up your appointment. A cell phone is best so you can get text messages.
We send a copy of your answers to this email right after you submit.
    Preferred language
Pick the language you are most comfortable speaking. We will try to send a notary who speaks it.
    2Your property and claim
This is the home that was sold, not where you live now. Use the address on your agreement with us.
It looks like FRI-CV-12 and is in the Funds Verified email we sent you. If you do not have it, leave this blank.
3.1How would you like to verify your identity?
    Choose one
A notary is a person licensed by the state to check who you are and watch you sign. You can meet at home, in a public place, or on a video call.
4.1How many people are on the deed?
    Count yourself
Everyone on the deed normally has to verify their identity too. If an owner has died, we will ask for a death certificate later. You do not need it today.
4.2Has your name changed since the deed?
For example by marriage, divorce or a court order
If the name on your ID is different from the name on the deed, we may need a paper that shows the change, like a marriage certificate.
5Which government photo ID will you show?

Do not type ID numbers and do not upload your ID. The notary checks your ID in person or on video at the appointment.

Pick the ID you plan to bring. It must have your photo and be issued by a government.
    Is this ID unexpired?
Look at the expiration date printed on the ID. Most notaries cannot accept an expired ID, so renew it before the appointment if you can.
6When would you like to meet?

These are requested times. We confirm the appointment with you after you submit. Pick a first choice and a backup. The earliest date is two days from today; times run from 8:00 AM to 7:00 PM, any day of the week.

    First choice

Tap the button, pick a day on the calendar, then tap a time. Gray days cannot be picked.

    Backup choice

If the notary cannot make your first choice, we try this one. Pick a different day or time.

    7.1Anything we should know?
Tell us about a wheelchair, hearing needs, a gate code, or the best time to call you. This helps the notary plan.
